Location details: The road may be un-named but is the northwards extension of Main St beyond the junction with Church La. The church is on the east side of the road and the memorial is on the eastern column of the north arcade.

Overall condition: Fair. The inscription is mostly legible.
|
Description:
An oaken board in the style of a triptych, though with fixed, rather than hinged, volets supported on brackets. The central panel has a moulded frame in the style of a gothic window, with a finial at the top and tracery in the head. Within the frame the gilded panel bears a black painted inscription in upright capital lettering with selected rubricated initials. The volets are slightly smaller and lack the finial, terminating in a cusp instead, and bear the names painted in black upright capital lettering.
Below the memorial are photographs relating to H D Heintz.

History
|
17th April, 1951: The church lies in the Peak District National Park designated on that date.
12th July 1967: The church was listed Grade II*, Heritage List for England number 1109264.
27th July 2016: The memorial was surveyed by Roy Branson and Alan Willmot. It was in fair condition but the surfaces of the two volets had deteriorated and were not easy to read.
