An oblong oaken board with a moulded cornice and broken pediment at the head, moulded borders at the sides, and a shelf at the foot. The inscription is painted in upright mixed case roman lettering.

The church lies within the Morley Conservation Area. 10th November, 1967: The church was listed Grade I, Heritage List for England reference 1205838. 13th May, 2009: The memorial was surveyed by Roy Branson and it was in good condition. 4th April, 2015: The memorial was photographed by Roy Branson and it was in good condition.
