Location details: This is the Roman Catholic Church at the western end of Church St near the junctions with Woodhead Rd (B6105) and Hall Meadow Rd, and is not to be confused with All Saints' Anglican Church at the eastern end of the same street. The tablet is inside the church on the east wall, north of the door. |

| Description: A wooden board constructed as a replica of the superjacent WW1 tablet. The surface panel is nowy-headed with hollowed shoulders, a small cross within a roundel painted at the head and a painted inscription in black upright capital block lettering. The panel appears to be mounted on an oblong panel serving as a backboard with the dates at the head. |
