Location details: The church is at the very end of the lane about 600m south of Dalbury Hollow, which is about 4.5km west along Radbourne Lane from Mickleover. The plaque is in the nave, on the north side of the chancel arch.

Description:
A rectangular brass plaque on a wooden backboard. The plaque has an incised border of acorns and oak leaves and the incised inscription is in upright capital roman lettering coloured black with some initials rubricated.

1929: The memorial was installed.
19th January, 1967: The church was listed Grade II*, Heritage List for England number 1334550.
22nd January, 2009: The memorial was surveyed by Roy Branson and Alan Willmot. It was in fair condition and well cared for.
